Job Title ;Palliative Care Liaison Nurse Department ; Hospital Wide Reports to ; Head of Cancer Services and Consultant Palliative Medicine Date ; 2023 Overall Purpose of Job The Palliative Care liaison nurse will work closely with the Palliative Care Team and Consultant in Palliative Medicine so that the Hospital can deliver a Palliative Medicine Service in line with our mission "to deliver exceptional patient care in an environment where quality, respect, caring and compassion is the core of our practice". This role will facilitate and support the implementation of evidenced based practice, and will contribute to the continuing development of the Palliative Medicine Service at Beacon Hospital. As outlined in the 'framework for the establishment of Clinical Nurse/midwife specialist posts' (National Council for the development of nursing and Midwifery, 2008), the Palliative liaison nurse will encompass a major clinical focus, which comprises of assessment, planning, delivery and evaluation of care given to patients and their families in the hospital and as outpatients. Key Responsibilities and Deliverables Provide specialist knowledge, expertise and care to palliative medicine patients. Disseminate nursing research and audit and provide consultancy in clinical practice to their nursing colleagues and the wider interdisciplinary team. Provide continual optimal care for inpatients, out-patients and their families. Assess, plan implement and evaluate all aspects of patients' physical, psychological, and social needs. Monitor patients on an ongoing basis, to evaluate outcomes and adjust nursing care practice to ensure optimum co-ordination and delivery of care. Carry out daily rounds on all palliative medicine patients on all floors Develop and participate in palliative medicine clinics Keep accurate and legible records and be aware of the legal implications of these documents. Liaise closely with patients' primary health medicine teams and review all referrals in collaboration with Palliative Care Team and Consultant in Palliative Medicine. Participate in multidisciplinary meetings and act as patients' advocate To monitor and ensure effective discharge planning for patients with complex palliative medicine needs Participate in family meetings and provide support as appropriate Promote multidisciplinary and partnership working in conjunction with ward based teams, community teams, patients and their carers. Develop care pathways appropriate to the acute palliative medicine setting Act as a resource and role model for specialist practice. Participate in the management of a caseload of patients and associated responsibilities Participate in the development and implementation of policies, protocols, and guidelines for palliative medicine patients. Identify service and learning needs on an ongoing basis Personal and Professional Development Develop the required skills and knowledge to support service needs within the scope of professional practice Undertake training to enhance nursing practice within the specialist area Identifies own continuing professional development needs and engages accordingly Act as a resource to colleagues To ensure and take responsibility for own professional practice and knowledge. Education Provide education for patients and their families Ensure adequate patient education material is available and update as required Incorporate health promotion principles into all aspects of nursing care Participate in teaching programmes and in-service training programmes for staff Recognise the opportunity for clinical supervision and access if appropriate Quality Assurance, Audit and Research Participate in and audit the clinical effectiveness of the acute palliative medicine services in conjunction with the palliative care team and Consultant Palliative Medicine and members of the Medical Oncology Team. Ensure that the relevant statistics are collated, updated and highlighted, pertinent to individual caseloads and referrals to the service. Ensure that clinical practice, procedures and standards of care are evidence based and remain dynamic, ensuring quality service development to meet patients' needs. Encourage, initiate and participate in nursing research where appropriate. Evaluate findings and assist with implementation of changes to improve the service Maintain awareness of current developments in the area of Palliative Medicine Contribute to service planning through use of audit, data and specialist knowledge. Comply with National Guidelines/Report on Palliative Care Services in Ireland Person Specification Qualifications ABA registered without any restrictions to practice Recognised additional relevant qualification Experience (Amount and Type) 5 years post registration experience 2 of which must be in the speciality area of oncology and/or palliative medicine Job Specific Competencies and Knowledge Interest in acute palliative medicine and development of service for the future Clinically competent in the care of Oncology patients Personal Competencies All posts in Beacon Hospital require a high level of flexibility to ensure the delivery of an effective and efficient service. Therefore, the post holder will be required to demonstrate flexibility as and when required by their manager of hospital management.
